Bad UI leads to someone trading $2m in crypto for 5 cents. There was a trade they wanted to make, the platform displayed “Locking in this rate…” and then failed to do so, instead setting up another trade. A pop up says “do you want to trade 2m for 5c?” and the user clicks through.

Somehow they’re blaming the user for trading without reading the text of the pop up that appeared after they promised to do the trade the user requested.
